Jack's parents also tend to run out of the house with just a cup of coffee to start their day. For lunch, he eats foods purchased at the cafeteria such as chicken nuggets and hash brown potatoes along with an orange, a chocolate chip cookie, and a carton of whole milk. In addition, Jack's school, like some other schools in the United States, has vending machines filled with high-calorie snacks and sugar-sweetened beverages, with no healthy options for students to purchase. Both of these snack options are packed with empty calories and are nutrient poor. He eats these snacks while watching cartoons or playing video games. Jack does not participate in after-school activities that involve physical activity, and because his parents often come home late from work, Jack is looked after by a babysitter who pays little attention to what he eats or his excessive screen time. Jack's mother works a long day and to save time, she often picks up dinner from a fast food restaurant. Dinner may include hamburger and fries or Chinese or Mexican food. Such foods, purchased at fast-food establishments, generally consist of oversized portions and excessive empty calories. This label gives the amount of calories, fat, and other nutrients in the recommended serving size. When managing calories, it is important to consider how much food is actually consumed by an individual compared with the recommended serving size on the label. Often the amount consumed may be more than the recommended serving size. Calorie and nutrient values need to be adjusted to reflect the actual amount consumed by an individual. They now notice that calories can add up quickly, especially because many of Jack's favorite foods and typical meals are packed with empty calories.

Jack needs to choose his foods wisely to stay within his energy and nutrient goals while still feeling satisfied so he is not hungry. As part of the AAP Expert Committee's stage 2 Structured Weight Management approach, Jack's pediatrician refers Jack and his mother to a registered dietitian, who will be able to spend time educating them and suggesting healthy meal options that Jack could eat at school and that the family can enjoy together at home. To get to a healthy weight, Jack needs to make food choices that are filling while eating fewer calories than he is currently consuming. Children can also become involved by helping in the kitchen, setting the table, and cleaning up. Excess screen time (sedentary activity) makes maintenance of energy balance difficult. Having a television in bedroom has been shown to be associated with increased screen time for children.Jack's parents remove the television from his room. Jack now wakes up with enough time in the morning to have breakfast with the family rather than eating on the run. A quick and easy breakfast, such as cereal, fat-free milk, and fruit, provides a convenient way to consume important nutrients.

Fat-free milk is used in place of whole milk as a source of calcium and vitamin D without the saturated fat and extra calories. Jack's mother also prepares a packed lunch for school, consisting of a sandwich with whole-grain bread, lean turkey, and low-fat cheese. Based on the USDA's MyPlate recommendations, fruits and vegetables make up about half of Jack's lunch, with the other half containing whole grains, lean protein, and low-fat dairy. Jack has enough food to feel full and can avoid the high-ED snacks and sugar-sweetened drinks from the vending machines. Jack may get a piece of low-fat string cheese or yogurt, a piece of fruit, or a serving of raw vegetables with low-calorie dip. Providing a variety of tasty vegetables encourages Jack to try new vegetables and to increase his vegetable intake. At first, Jack complains that he prefers cookies and chips, but Jack's mother has stopped buying these higher-ED options and provides guidelines to Jack's caregivers. After adjusting to the new foods over time, Jack begins to identify fruits and vegetables that he enjoys eating. She serves whole-grain pasta with a tomato-based sauce. This recipe is quick, easy to make, and includes lots of pureed vegetables that reduce the ED and increase the vegetable content of the meal. To save time, she uses vegetables she pureed over the previous weekend and kept in the refrigerator. She serves other low-ED side dishes that may include soup, salad, steamed vegetables, or fruit such as (unsweetened) applesauce. Fat-free milk and water are served with dinner instead of juice or soda. If he sticks with these strategies, he is headed for healthy growth and further improvement in his weight status over time. Jack and his mother continue monthly follow-up visits with his pediatrician to ensure that they maintain the healthy habits that have led to Jack's success thus far.
